To request a change to your benefits after the enrollment period, review the guidelines listed on the Submit Enrollment Changes page.
Teammates in positions classified at 24 hours or more standard hours per week are eligible for medical and dental. Positions classified at 16 or more standard hours per week are eligible for vision, flexible spending accounts and Additional Benefits.* If you are unsure, review the Benefits Eligibility Guide.

Eligible Dependents include your:
Documentation is required to verify eligibility of all dependents being enrolled in the medical, dental and vision plans. It is considered fraudulent to cover non-eligible individuals on your plans. A request to verify new dependents will be mailed to homes from Dependent Verification Services.
To qualify as an eligible dependent for a Health Savings Account (HSA) or Flexible Spending Account (FSA), dependents must be claimed as a dependent on your tax return.

Be sure to have the Social Security Numbers and birth days for any eligible dependent(s) you plan to enroll.

Important: You will receive a message that your elections have been submitted. Be sure to print your Benefits Enrollment Summary for your records.
Proceed to enroll in Additional Benefits offered.

The day after you complete the Enrollment Process, you may view your Benefit elections within YourHRLink.
Once your enrollment period closes, your benefit choices for your New Hire Event are final and you will not be able to make changes unless you experience a qualifying life event.
